On the rare occasion that a breast surgery complication does arise — for example, an implant ripples or capsular contracture develops — Dr. Ary Krau can perform revision breast surgery. One of the tools he uses in complex or tricky breast revision cases is a biomaterial called acellular dermal matrix.

What Is Acellular Dermal Matrix?

Think of acellular dermal matrix (ADM) as “scaffolding” for the breast tissue. It is a lattice-like skin graft from which the cells have been removed. ADM is placed within the breast to support the natural tissue; over time, your own cells grow into the framework and gradually your tissue replaces the graft. This provides a thicker and stronger layer of tissue than what would have developed without the ADM. Your blood vessels also grow into the material so it has its own blood supply.

Once placed, you cannot feel ADM. It takes about six months for the body to absorb the material.

Studies have shown excellent revision breast surgery outcomes with ADM. While it is almost always used in breast revision operations, it may occasionally be used in primary breast surgery, if Dr. Krau deems it medically appropriate.

Capsular contracture is a condition in which the breasts become hard, deformed and uncomfortable. Manage Implant Rippling

ADM can be used to disguise implant rippling (more common in saline implants) in patients with thin skin. The graft is placed over the implant to add more of a barrier between the implant and the skin.

Avoid Yo-Yo Dieting. Extreme fluctuations in weight can cause the skin — including breast skin — to lose its elastic quality, so it can no longer “bounce back” to a smaller shape and size once you lose a substantial amount of weight. Try to maintain a healthy weight by eating well-balanced, nutritious meals and getting regular exercise.

Don’t Smoke. We all know that smoking is bad for our health. But you may not realize how bad it is for your outward appearance as well. Smoking impedes proper blood circulation and robs your blood of life-giving oxygen. It also damages collagen and elastin, two proteins that are essential for healthy, elastic skin. All these negative effects of smoking can lead to the development of wrinkles, sagging skin and hard, dull-looking skin — throughout your face, décolletage and breasts.

Do Eat Nutritious Foods. Eating the wrong foods, such as excessive amounts of sugar, can wreak havoc on your skin, including your breast skin, making it look dull and dry. However, eating nutritious foods can have a beneficial effect on your appearance. For instance, eating foods high in antioxidants will help combat free radicals that damage skin cells and encourage cellular repair, both of which will help keep your breast skin looking healthy and youthful.

Don’t Go Out in the Sun Without Proper Protection. The skin on your neck and décolletage is thin and sensitive. Also, your chest area gets a lot of sun exposure when you are wearing bathing suits or low-cut blouses and dresses. Sun exposure without proper protection (i.e., clothes or broad-spectrum sunscreen with a high SPF) can cause dark spots (hyperpigmentation) to appear on your décolletage and cleavage wrinkles to develop. To keep your breasts looking young, don’t skimp on the sunscreen, and apply it liberally to your décolletage when you know you will be out in the sun.

Do Stay Active. Staying active will help keep you fit on the inside and looking great on the outside as you age. This includes cardiovascular exercise as well as strength training. Enhancing the muscles in your chest can provide a subtle lift throughout the breast area. Also, building muscle throughout your back, chest, arms and core will help you maintain good posture — which makes you look slimmer and your breasts look perkier. What woman doesn’t want that?

Bottoming Out

Patients who have thin or stretched skin may experience a loss of the skin’s natural elasticity, resulting in skin that is extremely loose or lax. These patients are susceptible to bottoming out, a condition that occurs when the envelope supporting the breast implant drops, causing a sagging, pendulous appearance. In these patients, the nipple ends up siting high on the breast with the bulk of the breast tissue hanging well below the nipple. The breast skin may also appear stretched below the nipple, magnifying the breast’s droopy appearance. In these patients, ADM can be used to provide a supportive meshwork between the breast tissue and the chest muscle, keeping the breasts in a more desirable position.

Capsular Contracture

Contracture occurs when unnaturally thick scar tissue forms around the implant, squeezing the implant over time and causing tightness and other changes in the appearance of the breast as well as discomfort in many patients. ADM has been shown to help thwart the formation of unnaturally thick scar tissue that causes contracture. Although it’s not entirely clear how it “shorts out” the scarring process, some experts believe ADM may help reduce inflammation that causes scarring; by promoting the rapid growth of healthy cells, ADM also may help “crowd out” scar tissue cells, preventing the formation of thick, fibrous tissue.

Rippling/Wrinkling

Wrinkling and rippling typically occur when the pocket formed to surround the implant stretches, becoming too large for the implant and allowing it to move or migrate from the desired position. The laxity that occurs can cause the tissues surrounding the breast to become loose, eventually resulting in wrinkles Wrinkling also can occur in women whose breast tissue is weak or thin. ADM is used to provide a supportive sling for the implants; over time, the patient’s own cells will form within the matrix, increasing support.

Fold Malposition

The natural folds along the bottom and side of the breasts are an integral part of the body’s natural curves. In some patients, especially those with thin or weak breast tissue, the pockets supporting the implants can become compromised, allowing the implant to drift downward or sideways, causing folds to flatten out and resulting in an unnatural appearance. ADM acts as a “tissue buttress,” providing added support for the pocket and redefining the natural folds.

Although thinned or weakened tissues typically are most common among older women and those who’ve had significant weight gain and loss, even younger patients can be at risk – especially those who have already had implants that may have stretched or otherwise overtaxed the supportive tissue, women who’ve had complications following implant surgery and those who’ve had a traumatic injury involving the chest area.

In the past, these patients likely would not be considered good candidates for augmentation or revision surgery. But now, thanks to a natural, biologic material called acellularized dermal matrix (ADM), patients with thin or weak tissue can experience the same results from surgery as those who have thicker, stronger underlying tissue.

What is ADM?

ADM begins with healthy cells that are “decellularized,”which means the essential materials that comprise and define the cell are removed. What’s left is the matrix – a framework or scaffolding that the body can use as a supportive skeleton to help new cells form.
